Beginning April 2022, as members of the Rotary family in District7030 and supporters of Polio Eradication – Rotary’s #1 Priority – you are invited to support the
Countdown to History and become a member of the new
District 7030 PolioPlus Society.
To become a member of the District 7030 Polio Plus Society, you are asked to commit to making an annual donation of US $100 (or more) to The Rotary Foundation’s PolioPlus Fund until such time that Polio is completely eradicated.and the World Health Organization certifies the world free of Polio.
Donations should be
made online prior to your application to become a member of the PolioPlus Society via
- Login to My Rotary (where possible) to ensure your contribution and foundation recognition points are counted towards your name
- Go to https://www.endpolio.org/donate or https://my.rotary.org/en/donate
- Ensure your select the Polio Fund
- Enter all necessary information
- Set up a recurring donation if desired so you do not forget each year
- Please save the payment confirmation number upon completing your contribution.
Once your contribution has been made, you can proceed to
register below to become a member of the District 7030 PolioPlus Society using this portal.
For your contribution and annual commitment, you will receive a District 7030 PolioPlus Society certificate and a PolioPlus Society pin, which will be mailed to your Club President for presentation at a club meeting or event.
Persons who make their contribution and register for membership in the District 7030 PolioPlus Society between 1st April and 30th June 2022 shall be specially recognized as
charter members of the society.
Please note th e following:
- Any one-time/non-recurring contriibutions made to the PolioPlus Fund and/or to any other fund of The Rotary Foundation (e.g. Annual Fund/SHARE) during or prior to the current Rotary year and/or prior to 25th April 2022 shall NOT count towards your qualification or eligibility towards becoming a member of the District 7030 PolioPlus Society – only recent contributions made after 25th April 2022 or recurring contribution prior to 25th April shall be recognized.
- You must provide your payment confirmation number received from The Rotary Foundation upon making your contribution
- To maintain your status in the District 7030 PolioPlus Society, you will be asked to provide a copy of your contribution confirmation from The Rotary Foundation once every year on on or before 15th July.
- Contributions to the PoloPlus Fund made in the name of a club or other organization DO NOT count towards an individual's qualification or eligibility for membership in the District 7030 Paul Harris Society
- Contributions to the PoloPlus Fund made via cheque, direct deposit and/or wire transfer shall be recognized only if proof of contribution is provided in the form of a contribution confirmation number.

- Mettez en place un don récurrent si vous le souhaitez pour ne pas oublier chaque année
- Veuillez enregistrer le numéro de confirmation de paiement une fois votre contribution complétée.
Une fois votre contribution effectuée, vous pouvez vous inscrire ci-dessous pour devenir membre de la District 7030 PolioPlus Society en utilisant ce portail.
Pour votre contribution et votre engagement annuel, vous recevrez un certificat de la société PolioPlus du district 7030 et une épinglette de la société PolioPlus, qui seront envoyés par la poste au président de votre club pour présentation lors d'une réunion ou d'un événement de club.
Les personnes qui apportent leur contribution et s'inscrivent pour devenir membres de la société PolioPlus du district 7030 entre le 1er avril et le 30 juin 2022 seront spécialement reconnues comme membres fondateurs de la société.
Veuillez noter ce qui suit :
- Toute contribution unique/non récurrente versée au Fonds PolioPlus et/ou à tout autre fonds de la Fondation Rotary (par exemple, Fonds annuel/SHARE) pendant ou avant l'année rotarienne en cours et/ou avant le 25 avril 2022 NE DOIT PAS comptent pour votre qualification ou votre éligibilité pour devenir membre de la société PolioPlus du district 7030 - seules les contributions récentes effectuées après le 25 avril 2022 ou les contributions récurrentes avant le 25 avril seront reconnues.
- Vous devez fournir votre numéro de confirmation de paiement reçu de la Fondation Rotary lors de votre contribution
- Pour conserver votre statut au sein de la Société PolioPlus du district 7030, il vous sera demandé de fournir une copie de votre confirmation de contribution de la Fondation Rotary une fois par an au plus tard le 15 juillet.
- Les contributions au Fonds PoloPlus effectuées au nom d'un club ou d'une autre organisation NE COMPTENT PAS dans la qualification ou l'éligibilité d'un individu à l'adhésion à la société Paul Harris du district 7030.
- Les contributions au Fonds PoloPlus effectuées par chèque, dépôt direct et/ou virement bancaire ne seront reconnues que si une preuve de contribution est fournie sous la forme d'un numéro de confirmation de contribution.
