1. CONTACT INFORMATION: Scroll down on this page and enter the contact information. You MUST Select the applicable "group" below the contact fields. Please select ONE of the groups shown. If more than one group applies to you such as Rotarian and you are Club President select Club President.

2. MENU AND EVENTS SELECTION: On this page you will enter the quantities for each meal choice you wish to make next to your name and your guests name if applicable. Only enter a quantity for the meals you plan to attend.
