REGISTRATION FORM
YMCA of the Rockies -

June 7-9, 2012
Instructions:
1. Register yourself & if applicable your first adult guest
prior to registering additional guests or children. You should not register
guests from different age groups on the same registration. If you have
children who fall into separate age group categories you will need to complete
a registration for each child’s age grouping. Do not enter your name and your
child’s name on the same registration.
2. If you have a clubrunner account you may use the log-in
area to simplify the process. Otherwise just enter the required
information as requested. Be sure to select the appropriate group below
your registration log-in information.
3. To register additional guests or children you will need to
close the registration page and return to this form via the District Rotary Web
Page (do not use your “back” arrows) and complete it again for those additional
people without logging into your clubrunner account. Avoid receiving a
duplicate registration error by using a different valid email address than used
on your original registration. Or create an invalid email address by putting
the words ‘guest1of” or “guest2of” in front of your email address. For example Guest2ofLynnMatson@comcast.net You will need to print a copy of these registrations as
you will not receive an email for these invalid email addresses. Since
each additional registration is an independent registration, you will not
receive a summary statement. If you are paying by check you will need to
send a check for the sum total of your registrations.
4. Enter your
confirmation number from your previous registration, if applicable, in the area
where requested. This will enable registration to combine all members of your
party.
