Please complete the registration below. Registraion can be done individually for you and your guest. Or, Rotary Clubs have the option of purchasing a full table for 8 or a half table for 4. If this registration is being made for multiple members and guests, please make sure to include the names of each guest in the questionaire section of the registration. The names of guests must be listed in this registration form no later than Oct. 25th. In the event that we do not have names to fill all 8 (or 4) purchased seats after Oct 26th, the Foundation Gala committee reserves the right to reassign the unnamed seat if needed for late registrants.

To complete your registration, person registering must pay online by debit/credit card.

Clubs purchasing tables or half tables will be seated at reserved tables that comforatably seat 8. The location of the table will be determined by the time stamp of the registration. Those with the earliest registration date/time will have the closest proximity to the stage.

There are a limited number of "special rate" hotel rooms available in the adjacent Hilton Garden Inn. The cost of hotel rooms are not include in this event registration. Attendees desiring a hotel room can call the hotel directly at 817-281-5800 to make the reservation and pay for the room.

Registration for this event closes October 25th at midnight.
